M. TUBERCULOSIS DISEASE:

M. Tuberculosis (TB) Infection is transmitted from person to person by airborne infectious particles called droplet nuclei. Chronic lung infection is the most common clinical presentation of TB.

TUBERCULIN SKIN TEST:

The Tuberculin Purified Protein Derivative (PPD) skin test is used to screen and identify apparently well individuals infected with M. Tuberculosis. The PPD test will be used for TB skin test screening and is performed by the intradermal injection of 0.1 ML of Tuberculin PPD into the skin of the flexor surface of the forearm.

INTERPRETATION OF THE TEST:

The test should be read 48 and 72 hours after administration. Sensitivity is indicated by induration usually accompanied by erythema. Any palpable induration of 10mm or more is considered a positive reaction. Health care workers who test negative and who cannot document a history of a negative tuberculin skin test within the past year should have a second skin test within 1-3 weeks to establish a true baseline skin test.

I have been informed that CDC guidelines require screening for possible tuberculosis. I agree to the use of the tuberculin PPD skin test. I have been informed of the possible side effects.

If I have a positive reaction to testing or have had previous positive test results, I agree to obtain chest x-rays if requested. I agree to seek medical attention from my attending physician or other health care provider as recommended by the local health department, should it be requested by the director of Clinical Services.

Treatment is mandatory, proof of treatment must be provided to the Director of Clinical Services. Refusal of treatment will be grounds for termination of employment with Aurora Hospice Agency.
